At Hazel Hawkins Memorial Hospital, we operate two skilled nursing facilities to help our patients recover when they need inpatient rehabilitation. These facilities are staffed by an on-duty registered nurse 24/7, with other clinical staff on-site every day. Other members of the clinical team include a registered dietitian, social worker, and an interdisciplinary team. The objective of our interdisciplinary team is planning the care each resident needs to feel their best, medically, psychosocially, and spiritually.
