Patient Portal - Request for Medical Records
Hazel Hawkins Memorial Hospital now offers you secure online access to your medical records.
Using a secure username and password, patients can view health information such as:
- Recent doctor visits
- Discharge summaries
- Medications
- Immunizations
- Allergies
- Lab results
- Radiology reports
- Surgical reports
- Appointments
- Pay your bill
Enrolling in the Patient Portal also gives you the ability to print out your own medical records.
How do I sign up?
- Your email needs to be on file with the hospital to enroll in the Patient Portal.
- Please contact PatientPortal@HazelHawkins.com to verify your identity to enroll in the portal if you do not have an email on file with the hospital.
- Proxy Access Patient Portal - This form is used to request access for children under the age of 12.
- Patient Portal Access for Minors - This form is used to request access for patients 12 - 17 years of age.
